Avacopan (Tavneos) is a complement 5a receptor (C5aR) antagonist, used as adjunctive therapy for severe active antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis in adults.

What Are the Side Effects of Avacopan (Tavneos)

Gastrointestinal Reactions

Nausea (23.5%).

Vomiting (15.1%).

Diarrhea (15.1%).

Neurological Symptoms

Headache (20.5%).

Dizziness (6.6%).

Paresthesia (5.4%).

Other Common Adverse Reactions

Hypertension (18.1%).

Rash (11.4%).

Fatigue (10.2%).

Serious Side Effects to Watch for with Avacopan (Tavneos)

Hepatotoxicity

The incidence of abnormal liver function tests and hepatobiliary events is relatively high.

It is recommended to perform liver function tests before treatment, and monitor once every 4 weeks for the first 6 months after treatment initiation.

Suspend treatment when ALT or AST > 3 times the upper limit of normal; discontinue treatment when ALT or AST > 5 times the upper limit of normal or accompanied by bilirubin elevation > 2 times the upper limit of normal.

Hepatitis B Virus (HBV) Reactivation

Screen for HBsAg and anti-HBc before treatment.

Patients with current or past HBV infection should consult a hepatologist.

Discontinue treatment immediately if HBV reactivation occurs.

Severe Infections

Pneumonia and urinary tract infections are the most common severe infections.

Avoid use in patients with active severe infections.

Closely monitor for infection symptoms during treatment.

Avacopan (Tavneos) Medication Precautions

Contraindications

Patients with severe hypersensitivity to avacopan or any of its excipients.

Active HBV infection.

Medication Use in Special Populations

Pregnancy: Sufficient studies are lacking; animal data suggest potential risks.

Lactation: Whether it is excreted into breast milk is unknown; the pros and cons should be weighed.

Hepatic Impairment: No dosage adjustment is needed for mild or moderate hepatic impairment; data are insufficient for patients with severe hepatic impairment.

Renal Impairment: No dosage adjustment is needed.

Swallow the capsules whole; do not chew or crush them.

Recognize the symptoms of allergic reactions and seek medical attention promptly.

Monitor for signs of abnormal liver function.

Be alert to infection symptoms.
