Dosing Guidelines

The treatment is initiated at a dose of 300 mg taken orally once daily, with or without food. Tablets must be swallowed whole. Following initiation, hemoglobin levels must be monitored every two weeks until stable, and at least monthly thereafter. Dose adjustments are made in 150 mg increments to maintain a target hemoglobin level of 10 to 11 g/dL, with a maximum dose of 600 mg. Dose increases should not occur more frequently than every 4 weeks. Administration should be staggered with oral iron supplements and phosphate binders.
