Ramelteon is a melatonin receptor agonist used for the treatment of sleep-onset insomnia.
I. Standard Dosage and Administration
1. Recommended Adult Dose:
(1) 8 mg once daily, taken within 30 minutes before bedtime.
(2) Total daily dose should not exceed 8 mg.
2. Administration:
Swallow tablets whole; do not split or chew.
3. Course Evaluation:
If insomnia does not improve after 7 to 10 days of continuous use, it may indicate another underlying medical condition, and the diagnosis should be re-evaluated.
II. Precautions
1. Avoid Taking with High-Fat Meals:
(1) A high-fat meal may increase total drug exposure by 31%, decrease peak concentration by 22%, and delay time to peak concentration by approximately 45 minutes.
(2) It is recommended to take on an empty stomach or after a light meal.
2. Contraindicated with Fluvoxamine:
Fluvoxamine (a strong CYP1A2 inhibitor) can increase ramelteon exposure by approximately 190-fold; co-administration is absolutely contraindicated.
3. Beware of Complex Sleep Behaviors:
(1) Complex behaviors such as "sleep driving," eating while asleep, making phone calls, or engaging in sexual activity may occur after taking the medication, often with subsequent amnesia.
(2) If any such behavior occurs, inform your doctor immediately and strongly consider discontinuing the medication.
4. Monitor for Allergic Reactions:
(1) Rare but severe angioedema of the tongue, glottis, or larynx may cause airway obstruction.
(2) If you experience difficulty breathing, a feeling of throat tightness, or nausea/vomiting, seek emergency treatment immediately.
(3) Patients who have previously experienced angioedema should not use this medication again.
5. Watch for Psychiatric Symptom Changes:
(1) Patients with pre-existing depression may experience worsening depression or suicidal ideation.
(2) If hallucinations, bizarre behavior, agitation, or mania occur, seek medical attention promptly.
(3) Avoid hazardous activities: The medication may impair mental alertness, and drowsiness may persist into the next day. Do not drive or operate heavy machinery until fully awake.
6. Reproductive Endocrine Monitoring:
If amenorrhea, galactorrhea, decreased libido, or fertility issues occur, prolactin and testosterone levels should be checked.
7. Warnings for Special Populations:
Use is not recommended in patients with severe hepatic impairment or severe sleep apnea; use with caution in patients with moderate hepatic impairment; safety and effectiveness in children have not been established.
III. Healthy Lifestyle Recommendations
1. Establish a Regular Routine:
Go to bed and wake up at fixed times each day to help stabilize your circadian rhythm.
2. Optimize Your Bedroom Environment:
(1) Keep the bedroom quiet, dark, and cool.
(2) Turn off electronic screens one hour before bedtime to avoid blue light interference with melatonin secretion.
3. Avoid Stimulation Before Bed:
Avoid vigorous exercise, watching intense or thrilling content, or handling work-related stress within 2 hours before bedtime.
4. Be Cautious with Diet:
(1) Dinner should not be too heavy; avoid drinking large amounts of water before bedtime to reduce nighttime urination.
(2) Finish main meals, especially high-fat meals, at least 2 hours before taking this medication.
