Recommended Dosage

Dosage varies by age and infection type. Adults and adolescents (≥12 years) typically receive 600 mg intravenously or orally every 12 hours for most indications, while the dose for uncomplicated skin infections is 400 mg orally every 12 hours. Pediatric patients (birth to 11 years) generally receive 10 mg/kg every 8 hours. For preterm neonates under 7 days, an initial regimen of 10 mg/kg every 12 hours is recommended. Treatment duration ranges from 10 to 28 days. No dosage adjustment is needed when switching from intravenous to oral administration.
