Contraindications and Medication in Special Populations of Pioglitazone Hydrochloride and Metformin Hydrochloride Tablets (Metact)

Clarifying the absolute contraindications and the populations requiring cautious use is the key to ensuring the safe use of this product. This product contains pioglitazone and metformin, each of which has its own therapeutic red lines and special considerations.

I. Absolute Contraindications

1. Contraindications Related to Cardiac Dysfunction

(1) Patients with current heart failure or a history of heart failure are strictly prohibited from using this product.

(2) Pioglitazone in pioglitazone metformin tablets can cause fluid retention, increase cardiac burden, and may induce or exacerbate heart failure.

2. High-Risk States for Lactic Acidosis

(1) Patients who have experienced lactic acidosis before are prohibited from reusing this product.

(2) Patients with severe renal impairment (eGFR below 30) or severe hepatic impairment are contraindicated.

(3) Patients with severe cardiopulmonary dysfunction, shock, pulmonary embolism, or other conditions causing tissue hypoxia are contraindicated.

(4) Patients with dehydration, such as severe diarrhea, vomiting, or difficulty in food intake, are contraindicated.

(5) Patients with excessive alcohol consumption are contraindicated, as alcohol severely interferes with lactate metabolism.

3. Other Explicit Contraindications

(1) Patients with type 1 diabetes, diabetic ketoacidosis, or pre-coma are contraindicated.

(2) Patients with severe infections, around major surgery, or with severe trauma should switch to insulin and are contraindicated.

(3) Patients allergic to any component of this product or to other biguanide drugs are contraindicated.

(4) Pregnant women or women who may be pregnant are contraindicated.

II. Special Population Considerations

1. Patients with Renal Impairment

(1) Strictly prohibited in patients with severe renal impairment or those on dialysis.

(2) Moderate renal impairment (eGFR between 30 and 60) requires extreme caution; when eGFR is below 45, use only if the benefit clearly outweighs the risk, with close monitoring of renal function.

(3) Mild renal impairment should be used under physician guidance with regular follow-up tests.

2. Patients with Hepatic Impairment

(1) Severe hepatic impairment is an absolute contraindication.

(2) Mild to moderate hepatic impairment requires careful use after physician evaluation, as reduced hepatic lactate metabolism increases the risk of acidosis.

3. Elderly Patients

(1) Elderly individuals have physiologically reduced renal and hepatic function, making them more susceptible to lactic acidosis.

(2) Patients over 75 years old require special caution; the decision to use and the dosage must be determined by a physician after comprehensive assessment.

(3) Even with normal serum creatinine, renal function should be assessed comprehensively using eGFR and other indicators.

4. Pregnant and Breastfeeding Women

(1) This product should be avoided throughout pregnancy.

(2) Breastfeeding women should, under physician guidance, weigh the therapeutic benefit against the benefit of breastfeeding to decide whether to continue nursing or discontinue the drug.

5. Children and Adolescents

The safety and efficacy of this product in pediatric populations have not been established; use is not recommended.

III. Use Restrictions in Patients with Coexisting Conditions

1. Patients at Risk of Hypoglycemia

(1) Patients with irregular eating habits, poor nutritional status, or pituitary/adrenal insufficiency have increased hypoglycemia risk and should use cautiously.

(2) When used with sulfonylureas or insulin, the risk of hypoglycemia rises significantly; blood glucose should be closely monitored and dosage adjustments may be needed.

2. Patients with Ocular Complications

Rapid reduction in blood glucose may cause transient worsening of diabetic retinopathy; patients with retinopathy should have enhanced fundus examinations.

3. Patients with a History of Bladder Cancer

(1) Patients currently receiving treatment for bladder cancer are contraindicated.

(2) Patients with a past history of bladder cancer should have a full risk-benefit assessment by a physician before use; during treatment, be alert for urinary symptoms such as hematuria and frequent urination.

4. Patients Using Concomitant Diuretics or Other Drugs

When used concurrently with diuretics or SGLT2 inhibitors, the risk of dehydration increases, thereby raising the risk of lactic acidosis; special attention to fluid balance is required.
